Pardon if I missed it or read over it, but are the traditional forms of the past- Shii Cho, Soresu, Ataru, etc- still being taught and practiced, or have they been lost?

I believe they are being used, so they're around.

They could, at least in theory. Lightsaber use doesn’t necessitate Force-use, as we saw with General Greivous. I’m wondering, though: do we have any specific examples of either schools or individual trainers still using the lightsaber forms in the sequel era? Anything precise?

There’s Wookiepedia info on Luke being a Form V user. Considering Obi-Wan was a master Form III user, we might surmise that the forms are still something that can be taught post Clone Wars either via sacred texts or perhaps by Obi-Wan himself taught Luke other forms outside of presumably Form I and Form III.

This isn’t solid info though and mainly conjecture.
